About the program:

Living with diabetes can be tough. At our Hospital, we’re here to empower you with the knowledge, confidence and support you need to take control and lead a healthy life. 

Who can benefit from this program?

This program is designed especially for individuals who are newly diagnosed with diabetes (type 1 and 2) and those struggling to manage glucose levels.

This program can help you to:

  • Learn to manage your glucose levels effectively
  • Gain insights into nutrition and healthy eating
  • Discover tailored exercise routines for your needs
  • Connect with a supportive community of fellow participants
  • Build the foundation for a healthier, happier future

The program aims:

This program is delivered in a day program setting, consisting of 3 x 2-hour sessions per week for up to 10 weeks (the number of sessions depends on your insurance cover).

We provide:

  • A Personalised Plan: We understand that every individual’s journey with diabetes is unique. Our program begins with a one-on-one consultation from our rehab consultant, who then works with our allied health team to create a plan based on your individual needs, challenges and goals. Our rehab consultant will then check in on you regularly to assess and measure your progress.
  • Exercise and Education: Our program combines exercise therapy in our large open plan gym and nutrition/heart health educational sessions to give you the skills to manage your diabetes effectively, beyond the program. 
  • Expert, Multidisciplinary Team: Our program is led by rehab consultants, cardiac nurses and allied health professionals including physiotherapists, exercise physiologists, occupational therapists and dieticians.
  • A Holistic Approach: We employ a holistic approach to your well-being, providing guidance around nutrition, medication and exercise.  We are dedicated to helping you build a healthier lifestyle for the long term.
  • You will receive personalised exercise therapy in our hydrotherapy pool and/or our modern gymnasium led by our allied health team.
How To access

Ask your GP, specialist or hospital (if you are an inpatient) for a referral to our program. Once we receive your referral, our team will promptly contact you to confirm any out-of-pocket costs and schedule your program start date.

Day Rehabilitation

Our day programs are designed for those living independently at home and seeking to improve their strength without the need for inpatient medical support. You can access our day programs either after an inpatient rehab admission or as required from the community.

Our day programs focus on improving your ability to complete activities of daily living and enhancing your overall quality of life. They typically involve 2-3 sessions per week, with each session involving 1-3 hours of exercise therapy based on your needs and goals.

Our day programs are suitable for those who:

  • Have completed an inpatient rehab stay and require ongoing rehabilitation support as they transition back home/into the community
  • Are focused on improving their fitness and mobility following an accident, injury or surgery
  • Would benefit from the support and expertise of our allied health team in managing a chronic illness

Inpatient Rehabilitation

Inpatient rehabilitation is suitable for individuals who require medical coverage and nursing care, and involves a full time stay at our hospital. As an inpatient, you are provided with a structured environment that helps you to focus on your recovery with regular mealtimes, bedtimes and daily exercise sessions with allied health professionals. This also ensures that you are closely monitored and changes in your condition are addressed immediately.

We provide medical inpatient services to patients who:

  • Are unable to cope at home or have no one to care for them after hospitalisation, after a bout of illness, a fall or fracture or those unsteady on their feet
  • Are suffering a chronic illness and are in need of acute hospital care
  • Are in need of continuing IV antibiotics or wound care such as VAC dressings
  • Are not ready for rehabilitation, for example, those who are non-weight bearing or slow to heal

The length of your stay will be dependent on your progress and your specific needs as assessed fortnightly by our rehabilitation physicians and allied health professionals.
